Student Health Insurance
2005-06
Columbia
College Chicago implemented a mandatory health insurance coverage
requirement in the Fall 2001 semester through Koster Insurance
Agency, Inc. Many colleges and universities in the United States
have made health insurance coverage mandatory at their institutions
since 1994. Koster Insurance Company will again be offering
an insurance plan for the 2005 - 2006 academic year.

Note:
ALL International students are AUTOMATICALLY enrolled
in the insurance plan. If you do not wish to be enrolled,
then you must petition for a “waiver” EVERY
SEMESTER that you are a student at Columbia College Chicago.

Cost
and Coverage
Coverage for the 2005 - 2006 academic year
begins on September 11, 2005, and ends September 11, 2006. The
cost is $803.00 for 12 months of coverage, payable in two installments
of $401.50 each. PLEASE NOTE that your OASIS account will be
billed automatically for the full amount. Even though you will
receive a charge for the entire amount, you will still be able
to make a partial payment of $401.50 without incurring any penalties!
When
to Pay
You must make two payments of $401.50.
The first payment, for the Fall 2005 semester, is due by Friday,
September 29, 2005. The second payment, for the Spring
2006 semester, is due by Friday, February 10, 2006.
How
to Pay
There are two ways to make your insurance payments.
• In person. Bring a check or money order payable to
“Columbia College Chicago” to the Cashier’s
Window located on the 5th floor of the 600 S. Michigan Avenue
building. Be sure to write your OASIS number on the check,
and indicate that it is a payment for “International
Student Health Insurance.” Keep the receipt for your
records.
Online. Visit
the CCCPay link on the Oasis portal homepage at www.oasis/colum.edu.
After you login to CCCPay, select Make Payment. Enter the
payment amount, payment type (eCheck or Credit Card) along
with the account and payer information in the appropriate
fields.
How
to Petition for a Waiver
If you do not want to be enrolled in the health insurance
plan, then you must petition for a waiver:
• You must submit a “Waiver Card” for EVERY
SEMESTER that you do not want to be enrolled in the health
insurance plan. Waiver cards are available at the Office of
International Student Affairs.
• You must provide proof that your coverage is comparable
to Koster’s plan, i.e. dates of coverage, benefits provided,
etc.
• The policy must be in English
• The insurance company providing the coverage must
have a U.S. address.
• The waiver card and supporting information must be
submitted to the Office of International Student Affairs no
later than September 23, 2005, for the Fall
2005 semester, and no later than February 6, 2006,
for the Spring 2006 semester.

International
Health Insurance for the spring 2006 semester has
the following changes:
Spring 2006 coverage begins January 23,
2006 and ends on September 11,
2006.

The cost of Spring 2006 coverage
(if you are not already covered through Koster
from fall 2005) is $543.00
PLEASE NOTE:
These changes are
specifically for those students who are going
to begin coverage through Koster in the spring
and NOT for students who began coverage in the
fall 2005 semester.
